11/8/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Broadnax indicated her day was well. Ms. Broadnax was well groomed and prepared for the session. Intervention: QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings, and emotions. QMHPA assisted Ms. Broadnax with increasing her social skills through using several exercises in which Ms. Broadnax must use her communications skills, listening, skills and responding skills daily. These skill building exercises will help her build her confidence so that she can begin to ask questions and respond openly and honestly in the home, community and on her job. QMHPA reminded, encouraged and supported Ms. Broadnax as she looked through her journal to explore her affirmations. QMHPA encouraged Ms. Broadnax to use positive eye contact, positive body languages, speak clearly and be assertive with respect to making sure her needs are met 3-5 times per week. This skill will help Ms. Broadnax learn to solve problems/negotiate. QMHPA encourages the Ms. Broadnax to be pro- active with making choices/decisions on how she allows others to dictate her day verse taking care of her needs and the needs of her family first. QMHPA used role-playing to assist the Ms. Broadnax in identifying positive ways she can express her feelings without feeling stressed or overwhelmed. QMHPA suggests decreasing her anger from 4-5 days to 2-3 days through using relaxation skills. Response: Ms. Broadnax appeared to be receptive to QMHPA’s interventions and services that were provided to her. Ms. Broadnax shared that she thinks she will have a good week because her attitude was better. Ms. Broadnax was able to identify how using appropriate social skills is helping her to increase the way she connects, gets information and is allowing her to develop relationships the people she is living with and they are making her stronger and realizing a lot of ways to reduce some of the stressors she has been experiencing. Ms. Broadnax was able to use the situational exercises to communicate her thoughts and feelings. Ms. Broadnax had to be redirected and encouraged twice to respond appropriately without using vulgar language. Ms. Broadnax shared that sometimes it is hard for her to avoid some curse words, but she would try. Ms. Broadnax shared that she often becomes anxious and often rushes her responses because she does not feel comfortable talking to most people. Ms. Broadnax had some difficulty speaking when faced with challenging questions. Ms. Broadnax admitted that she felt better when she was able to answer the questions openly and honestly to avoid being dishonest. Ms. Broadnax also shared that she felt she was making some progress with respect to building her self –esteem and her self-confidence through using her daily affirmations. Ms. Broadnax was able to process the benefits of journaling her feelings and using affirmations daily especially when needed. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
